The Chronic Care Program (CCP) provides care co-ordination and/or clinical support to people in the ACT and surrounds with Chronic Obstructive Pulmonary Disease (COPD), Chronic Heart Failure (CHF), Parkinson's Disease and other conditions by arrangement. It focuses on those people who may need assistance with managing their health and navigating the ACT health care system.
The CCP aims to:
- Encourage self-management at home or in community settings, including the use of peer-support and technology where appropriate
- Provide education and support to the patient, their carers and family, and assist with accessing appropriate health and community services
- Enhance communication among the Chronic Care Program (CCP) partners involved in the patient’s care, enabling more effective and timely monitoring and management of the patient’s condition
Eligibility criteria applies and referrals are required for inclusion in the program. Patients may be referred by specialists, GPs or other members of the multidisciplinary team when they identify a client who may benefit from any or all parts of the service.
