ACT Health - Community Care Nutrition Service
ACT Health provides free support for adults who would like to improve their nutrition and health. Their team of Accredited Practising Dietitians and Allied Health Care Assistants can help clients individually or as part of a group. They also offer home visits for those who are home bound.
Their individual appointments offer dietary assessments and advice or counselling for nutrition related needs that are associated with medical conditions, including:
- Cardiovascular disease
- Chronic diseases
- Malnutrition
- Minor gastrointestinal conditions
- Overweight and Obesity
- Palliative care
- Wound healing
